Patient Support Office
Introduction
In the patient support room, patients and their families can receive outpatient consultations, hospitalization and discharge, and post-discharge care in areas where they are accustomed to living. We will work with the hospital and the community to help you live a life of recuperation with peace of mind.
The work content is divided into (1) hospital admission and discharge support, (2) medical welfare
consultation, and (3) cooperation with hospitals.
(1) For hospital admission and discharge support, multi-professional collaboration from the outpatient
department so that patients can undergo hospitalization and surgery with peace of mind.We will provide
seamless and smooth support from hospitalization, discharge, and at home.
Patient Flow Management (PFM) system that consistently provides hospital admission and discharge support from
the outpatient stage.
(2) For medical and welfare consultations, social workers and discharge support nurses are available for
patients who require social support.
The nurses will consult with patients and their families about their concerns and worries about their daily
lives.
(3) In collaboration with hospitals and clinics, we will work closely with local medical institutions to carry
out forward collaboration within the region and provide an intensive play a role.

About support for admissions and discharges
About admission and discharge supportIn order to ensure that patients can receive medical treatment with peace of mind even before hospitalization, multi-professional collaboration is provided for scheduled inpatients to provide consistent hospital admission and discharge support from the outpatient stage (Patient Flow management ・PFM)。
・We provide various risk management and support at the outpatient stage before hospitalization.
・We will explain about anesthesiology examination, oral examination, internal medicine, nutritional status,
nursing care, etc.
About discharge support
We provide support for patients who have completed acute stage medical care so that they can live their own
recuperative life with peace of mind after being discharged from the hospital, even if they have an illness or
disability.
Cooperate with local hospitals, facilities, home-visit medical care, home-visit nursing stations,
etc.
2. About bed control
The head nurse in charge of bed control strives for smooth operation of the bed by adjusting the bed situation so that patients in emergency hospitalization can be admitted promptly.

We have established a relationship of trust with local medical institutions and have been working as a liaison with cooperating facilities.
4. About Medical Welfare counseling [click here for details]
A specialized medical social worker will think about and help the person and the family with the anxiety and worry caused by the illness.

We have a variety of consultations about cancer. In addition, we will cooperate with other departments as necessary in order to respond.
