What is Hospital and Clinic Collaboration, and what is its significance?

Content of the work that is done in the Hospital and Clinic Collaboration Office

1. For patients who have been referred by other institutions or facilities, we will report to the doctor who referred you that you have received the examination.
2. A patient's medical conditions, treatment policies, and clinical examination contents can be reported to the referring physician with the consent of the patient.
3. When patients say, "I want to change to another hospital but I don't know the appropriate hospital," or "I want you to refer me to a suitable hospital somewhere," we will cooperate with the Medical Welfare Counseling Office and consult with them.

4. We hold regular meetings with the local Medical Association, and we are working together to improve medical problems.
5. We hold a regular academic conference with the local Medical Association physicians and organize a meeting where doctors can study the advances in medical treatment that are constantly evolving.
   For example, (1) Kawagoe Clinical Conference, (2) Kawagoe City Medical Association Clinical Study Meeting, (3) Kawagoe Cardiovascular Meeting, (4) Kawagoe Diabetes Discourse Meeting and many other types of study meetings and study sessions are being held, and we are aiming to improve medical care in the future by participating in these projects in the Hospital and Clinic Collaboration Office.
6. We work with the doctors in all departments to cooperate with the Saitama Medical Center Visiting Nurse Station.　Information about the Visiting Nurse Station is listed separately.

FAX referral method

By FAXING in advance, we will prepare the referred patient's medical records beforehand.

Reception
9:00 - 17:00 (Monday to Saturday) until the day before the examination date * Please inquire before sending a FAX for the Department of Psychiatry, the Neonatal Department, the Department of Breast Care (Breast Oncology Department), and the Department of Gynecology (tumor outpatient) due to their all reservation system. We will make a confirmation call to your hospital after receiving the FAX. Please fill in your contact information and the patient's contact information (phone number).
