To those who are visiting the Saitama Medical Center

Requests regarding the handling of personal information

In order to perform medical care, we need to collect personal information such as the patient's medical history, medical findings, inspection samples, and inspection findings.

This center complies with the "Act on the Protection of Personal Information," and we pay strict attention to the handling of the information and make proactive efforts and establish rules in the hospital to protect personal information, regardless of whether the patient is alive or dead.

Thank you for your cooperation, and please understand the following about the purpose for use of personal information. These are based on the guidelines from the Ministry of Health, Labour and Welfare. Please contact the General Information counter and we can give you a printed copy if you wish.

  1. Purpose for use of personal information in this center See "Appendix." Unless we receive your intention of specific reservations, we will assume you agree and will use the information.
  2. If there is something that they do not agreed with, then the patient may be asked to confirm their clear intentions to this center in advance. In this case, please consult with the General Information counter.
    Incidentally, if you wish to consent or have reservations at a later date, you can change at any time.
  3. Please consult with the General Information counter if you ask to disclose, make corrections to, notify about the purpose of use, or to suspend use of your personal information. A fee will be charged in the case of a disclosure. Please contact the General Information counter if you have any questions or comments.

Purpose for use of personal information in this center
