Introduction

The mission of the Saitama Medical University is "Your happiness is our happiness." Our Center for Maternal-Fetal and Neonatal Medicine is one of the 5 Centers for Maternal-Fetal and Neonatal Medicine which were first approved in Japan in April 2000, and is the most historic comprehensive perinatal maternal and child care center in Japan. From April 2013, it was the largest in the world with 30 MFICU beds and 60 NICU beds.

Our center cooperate with the Department of Emergency and Critical Care Medicine, the Department of Obstetric Anesthesiology, and many other departments in Maternal Lifesaving project (if the mother's life is in danger due to obstetric hemorrhage, cerebral hemorrhage and other cerebrovascular disorders, heart failure, including perinatal cardiomyopathy, and injuries caused by traffic accidents, etc., and as a general rule we need to accept all cases 24 hours a day and 365 days a year) which was delegated by Saitama prefecture. We also support referrals of pregnant women who are in labor and postpartum with various complications and require emergency transportation, changing hospitals from the first medical facility or complete examination (for example, fetal diagnosis and fetal treatment are done in cooperation with the Department of Obstetric Anesthesiology, and the pediatric cardiologists group).

In addition, the mothers and babies who are admitted to our center, which is their last bastion in Saitama Prefecture, where high-risk pregnancy and labor are on the rise, have a great deal of stress with the risk to their life. We not only save their lives, but also support with a multifaceted approach to relieve the pressure felt by the mother, baby and their family by working together with all of the staff including clinical psychotherapists and social workers.

We have put forth our best efforts on providing advanced medical treatment for the mothers and babies that are high risk, as well as supporting them mentally so the mothers who come to our center can deliver and nurture their babies safely and comfortably and the babies can grow without any problems. We, the staff will continue to apply effort, aiming for a Center for Maternal-Fetal and Neonatal Medicine where we can provide them safe/secure and advanced perinatal care and where we can contribute to community medicine.

Patient care at our department

Maternal and fetal division:

Among the 944 annual deliveries (2016), referral cases and maternal transport cases such as pregnancy complications and fetal abnormalities exceed 60%. We work with the NICU to manage women with high risk pregnancies. The Center for Maternal-Fetal and Neonatal Medicine opened from 2000, and in 2013, we expanded to new wards making us the largest perinatal center in Asia. Because we deal with many high-risk pregnant women, the Cesarean operation rate is 59.3% (2016) and is one of the highest Cesarean operation rates for hospitals in Japan. Since 2008, we have been commissioned by Saitama Prefecture and have started the Maternal Lifesaving project. We also have clinical psychologists and we care not only for the physical side, but also from the mental side.

Maternal Emergency division:
In principle, all cases that require severe maternal critical care, such as massive bleeding and hypertensive disorders of pregnancy (HDP) during labor and postpartum. We serve as a critical care center dealing with obstetric shock.
Maternal Complications division:
We manage high risk pregnancies. We treat many cases of pregnant women with complications in cooperation with other departments. Counseling is provided during pregnancy, working on the prevention of postpartum depression, mental health care, and prevention of domestic violence and child abuse.
Fetal Diagnosis and Fetal Treatment division:
We have been focusing on fetal diagnosis and fetal pathophysiology under the direction of Dr. Baba, a leading doctor in three-dimensional ultrasound and one of the few ultrasound "instructors" in the obstetrics and gynecology field. We aim for consistent treatment, from the fetal stage to the neonatal period with the fetus as a "single patient," and appropriately select the treatment for newborn babies, including fetal treatment and neonatal surgery after birth, based on an accurate fetal diagnosis.

Neonatal division:

At the Center for Maternal-Fetal and Neonatal Medicine, which began operations in 2000, we are promoting high-quality neonatal medicine at a nationwide top-level with the objective of all newborns being healthy and living in a happy family. We are working to provide a wide range of warm medical treatment, for premature infants from the 22nd week of pregnancy, acute advanced lifesaving treatment of pathological newborns, to pediatric home care aiming for family-centered medical care in cooperation with the many professionals involved in obstetric as well as perinatal care. We provide medical care for high risk delivery assistance such as Cesarean operations, premature babies, and multi-embryo in cooperation with the Department of Obstetrics, medical care for more than 700 children transferred from inside and outside the hospital in need of NICU admissions and subsequent total care in the hospital, as well as daily visits with term newborns, and tracking the development of the children who were in NICU until after they start school. We focus on daily practice and research along with the promotion of neonatal medicine in Saitama Prefecture, as one of the leading facilities for newborn medical care in Japan while transmitting information to the nationwide General Perinatal Center.

Pediatric Cardiovascular division:

Our department has 3 pediatric cardiovascular specialists working on fetal diagnosis of congenital heart disease, diagnosis and treatment after birth, bridging to surgery, and exercise including other pediatric heart diseases and life management based on patient's life stage, with our basic stance of "Along with patients and the patients' families." We work closely with each division of the Department of Pediatrics, the Department of Obstetrics, the Department of Cardiology, Cardiovascular Surgery, and other related departments. We also conduct medical care for those who have had abnormalities indicated in school checkups as well as adult congenital heart disease. Please feel free to consult with us.

Anesthesia division

Our motto at the Department of Obstetric Anesthesiology is "We want to bring cesarean operations closer to the natural delivery experience." There are many lower body anesthetics that allow the mother to remain conscious, but general anesthesia can also be provided safely. A pregnant woman's anesthesiologist will help so that the baby can suckle at the mother's breast from during surgery, relieve the pain after surgery, and make it so that she can walk and breastfeed the baby during that day. Epidural painless delivery is also safely available 24 hours a day.

Maternal and Fetal division

Outpatient care reception including referrals are available on Monday through Saturday mornings excluding Sunday and public holidays (Reception: from 8:30am to 11:00am).

Referral patients will be seen for an examination between scheduled appointments. Please note that you may have to wait a few hours depending on the day.

Maternal complications with pregnancy:
We will manage pregnancies with the concerned department and conduct joint examinations in the case of pregnancies with maternal medical complications such as hypertension, renal diseases, blood diseases, diabetes, collagen diseases, or severe thyroid dysfunction, etc., or when combined with mental illness. We will proactively support the pregnancy cases which require professional delivery management by the Department of Obstetric Anesthesiology, such as scoliosis of mother, heart disease complications and post-surgery, cerebrovascular disease complications and post-surgery, and severe obese.
High risk pregnancy:
In addition to the above complications, we can accept cases where a previous pregnancy was high risk, such as multiple pregnancies or premature birth. Please call our outpatient regarding cases where it is difficult to decide whether to transport the mother, because we may ask for a referral with transportation of the mother in cases where hospitalization is required.
An ultrasound specialist will perform the fetal diagnosis. FAX reservations for ultrasound specialist outpatient are available on Mondays and Thursdays. It is also possible to be accepted through the usual referral support if a reservation cannot be made. We'd appreciate it if you drop us a line in advance, in addition to the FAX reservation, because it can be difficult to treat patients in the case of multiple fatal abnormalities when referred at the same time as normal referrals.
Maternal Lifesaving Center:
We support patients in cooperation with the critical care center, the Department of Anesthesiology and the Department of Cerebral Surgery in cases of obstetric shock such as abruption of the placenta, hypertensive disorders of pregnancy (HDP), puerperal hemorrhage, pulmonary embolism, disturbed consciousness, and cases that require multimodality therapy because obstetric shock is expected. When it is a severe case, please consult us on the dedicated line, saying, "This is a Maternal Lifesaving case," even though it is difficult to diagnose.

* In principle, we are not accepting referrals for those only desiring painless delivery, normal return home delivery, and those desiring only an amniotic fluid test, however we may be able to accept pregnant women with a low risk and pregnant women who strongly wish to deliver at our Center, so please let us know.

Neonatal division

The neonatal department staff consists of 20 people including professor Hisanori Sobajima, Deputy Center chief of Center for Maternal Fetal and Neonatal Medicine Kazuhiko Kabe, Instructor Akio Ishiguro, Instructor Masayo Kanai and 4 newborn specialists, along with 2 teams of doctors, and they provide medical care daily, centered in the NICU. We have been operating since 2000 as the only comprehensive perinatal center in the prefecture and have been promoting perinatal care while exchanging information closely with obstetrics and gynecology. In recent years, we have been in cooperation with maternal and neonatal transport coordinators in the prefecture accommodating cases. We have been mainly treating severe children, from super-pre-term infants in the 22nd week of the pregnancy, to mature children. Please consult the NICU Direct-dial phone (049-228-3890) for the hospitalization of a child born outside the hospital. The goal of perinatal medicine is for the children to live a happy life with their families. Discharge from the NICU is not the goal, but to track them as they grow, start school and continue to follow them after that, with feedback from the medical personnel and administrative staff who are together watching the growth in the region to take advantage of the advanced medical treatments in the field.

Pediatric Cardiovascular division

This division considers the examination and differentiation of minor symptoms which seem to be subtle for the possibility of heart murmurs, pulse irregularities, and other heart diseases to be a very important duty. Please consult with us about professional practice, such as the diagnosis and management of pediatric heart failure and arrhythmia, detailed evaluation by cardiac MRI, diagnosis of fetal congenital heart disease and detailed explanation to the family. We will support you with all of our capability.

Anesthesia division

If there is an anesthesiologist in the delivery room or the maternity ward, the anesthesiologist can immediately respond to an emergency Caesarean incision or a sudden change in the pregnant woman. As the first perinatal anesthesia division in Japan, our department is responsible for Cesarean anesthesia, rapid pregnancy support, painless delivery, anesthesia for collection of eggs with infertility, and neonatal anesthesia, aiming to be model of the anesthesia department in the Perinatal Center. We are also concentrating on the study of the maternity anesthesia field and the development of specialist physicians through international exchanges. Please consult with us as a pain clinic for pregnant women.
